Digital services commonly record the same evolving incident several times and in different formats. We developed a fact-centered architecture that converts service requests, system logs and video-event summaries into a common evidence-bearing lifecycle managed by Insert, Link, Merge, Update and Close. The original 20,000-record NYC 311 run reduced active repository cardinality by 68.41%, but that reduction alone did not establish merge correctness. On a separate 300-pair consolidation audit, the refined matcher reached 100.0% precision, 99.33% recall and 99.67% F1. Tests on 2,000 BGL log lines
produced 216 Updates, while 8,908 ETH trajectory observations produced 8,548 Updates. In a six-agent in-process retrieval test, contacting four agents preserved 97.87% Recall@5, reduced messages by 33.33%, and lowered measured query latency by 22.73% relative to broadcast. The experiments show that
heterogeneous streams can share one traceable repository mechanism without discarding source evidence.
